Who qualifies for SCN services?

Thrive Care LLC serves individuals and families who qualify through participating Medicaid programs and community support initiatives across New York State.

General qualifying criteria

  • New York State resident
  • Active Medicaid enrollment with a participating plan
  • Identified need related to housing, food, transportation, employment, or care navigation
  • Willingness to engage with a care navigator for assessment
