Who qualifies for SCN services?

Thrive Care LLC will serve individuals and families who qualify through participating Medicaid programs and community support initiatives across New York State.

General qualifying criteria

  • New York State resident
  • Active Medicaid enrollment with a participating plan
  • Identified need related to housing, food, transportation, employment, or care navigation
  • Willingness to engage with a care navigator for assessment

What happens after you apply

    Initial review

    We review your waitlist submission within 1–2 business days and confirm SCN eligibility when referrals open.

    Care navigator outreach

    A navigator contacts you to complete a personalized social needs assessment.

    Connection & follow-up

    We connect you to services and stay in touch to make sure they work.
